1. Industry Type: A dropdown list of major industry types is available to choose from, if your industry type is not represented select other and fill in your industry
2. Number of physical work facilities that will participate. How many buildings or offices do you have within the jurisdiction that you would like to participate in the program?
3. The total amount of Full Time Employees at each facility.
4. The names, e-mail address and primary work location of each employee who will receive a CEAS card. These employees can be uploaded at a later time into the CEAS system individually or in bulk
5. A photo of each employee who will receive a Standard, Multi-Facility or All Area Access card (see Card Types and Pricing). In most cases photos taken by your security department for your company ID are acceptable. Photos can be uploaded in bulk as long as each file name has an accurate identifier to individual.
